Summary
The AMA team came to us with a powerful goal which was to build a digital platform that keeps caregivers and patients closely connected, especially those living with chronic conditions or limited mobility.
We designed an experience that helps patients request care, follow simple daily plans, chat with caregivers, and stay on top of their health. With wearable integration and real-time updates, caregivers can monitor wellness and respond faster when it matters most.
On the backend, we crafted tools that make it easy for care teams to coordinate, schedule, and stay in sync — creating a smoother, more supportive care experience for everyone involved.

The first challenge we identified was how fragmented the current home care experience was — families and patients often used a mix of spreadsheets, calls, and messaging apps to manage care. This created confusion, inconsistent follow-ups, and stress for everyone involved.

There was little to no visibility into the actual progress or day-to-day changes in a patient’s condition. Caregivers would often keep manual notes, which rarely made it back to doctors or family members in any meaningful way. Without a structured way to log, share, and act on care data, opportunities for early intervention or better decision-making were often lost.

We designed the care request experience to be intuitive and frustration-free. Instead of relying on phone calls or texts, which can often be missed or delayed, patients can now request help directly through the app. The flow guides them step-by-step to specify what kind of support they need (medical, physical, emotional, etc.), who they need it from, and when. This improves response time, reduces ambiguity, and ensures that the right caregiver is dispatched quickly.
